What is testosterone?

Testosterone is a natural hormone produced in the body. Testosterone production in men is high during puberty and can affect a man’s muscle mass, hair growth and sex drive.

Over time, testosterone levels gradually decrease with age. Men with low testosterone, also called hypogonadism, can have a variety of symptoms.

What are the symptoms associated with low testosterone?

There are many symptoms associated with this condition. They may include some of the following:

Decreased sex drive
Loss of sleep
Hair loss
Mood swings
Fatigue
Depression
Increased weight gain
Decreased tolerance for exercise
Experiencing weak erections
Erectile dysfunction
Osteoporosis
Low energy
Reduced muscle tone
Men may experience many of the above symptoms and may only have a few signs. How many symptoms of low testosterone you experience will vary from patient to patient.

How common is it?

Although many men experience these symptoms, they do not realise that it is part of ageing. Although it is part of ageing, many men do not feel comfortable talking to their doctor about their symptoms and how they are feeling.

It is estimated that almost half of men over 45 and a third of men over 70 have low testosterone.

How do I know if I have low testosterone?

There is a simple blood test to determine testosterone deficiency. Low testosterone in men is often underdiagnosed. It may even be due to other conditions such as medication side effects or mental health problems.

What are the treatments available?

The most common treatment is testosterone replacement therapy. Testosterone replacement therapy is safely administered through implantable pellets or injections.

Our patients have been successfully treated with the long-acting depot shot, the long-acting testosterone injection Nebido and the testosterone gels (Testogel). Occasionally a patient uses nasal sprays or a pill, which are emerging treatments.

Are there any risks associated with treating low testosterone?

Men undergoing testosterone replacement therapy should be monitored for changes in blood thickness or haematocrit and prostate specific antigen (PSA) levels. They need to have a blood test between 2 and 4 times a year and prostate cancer screening every six months.

Testosterone therapy can lower sperm count and make men sterile during treatment. Men should therefore not use it if they are trying to have children. Other therapies may be prescribed for men who want to have children.
